1 In this regulation:
"Act" means the Health Act;
"agency" means the British Columbia Cancer Agency;
"information" means information or classes of information relevant to medical research into the prevention, causes, diagnosis, treatment and outcomes of cancer or its precursor lesions and includes records containing that information.
2 Information that may be requested by the agency pursuant to section 9 (1) of the Act is limited to information listed in Appendices 1 to 3.
Personal Identification Information
1 Personal identifiers
(a) name;
(b) sex;
(c) birthdate and place;
(d) marital status;
(e) residence information.
2 Family identifiers
(a) spouse's name.
3 Health identifiers
(a) health insurance number;
(b) personal health number.
Medical Information
1 Records from medical laboratories, imaging services, hospitals and other health facilities and physicians.
2 Mortality and morbidity data including autopsy reports.
Factors Which Influence the Occurrence of
Cancer in Human Populations
1 Intrinsic host factors
(a) ethnicity;
(b) medical history.
